Horse Vet Out Form

For use of the Zone and Senior Umpires as well as Horse Welfare Officers when a horse is vetted out. This should be used for accident (during or outside) of a game, injury (during or outside) of a game or illness.

If a horse is seen to be showing a Vice and has been removed or warned during a game then please use the Horse with Vice Form.
